Which forms do I need to print to file an application for my child's dental treatment through the Dental Care for Children with Special Needs Program?
All forms must be completed and submitted to the local Grotto representative. To find the closest Grotto representative, click here for Dental Program Contact Information and indicate your State. Upon completion of the forms, the application and dental pre-treatment estimate must be sent to that representative. He will sign your forms and send them to the Humanitarian Foundation office for processing. Upon receipt of your application, the office will process it and send a direct communication to your dentist. The local representative will receive an update on the status of your application and you can contact him if you have questions. When there is no representative close to your area, you will be instructed where to send the application.
